error 2753

[Salamander]

I have an install that I have been deploying for some time. All the sudden I started getting the error -2753 on one of the custom actions that I run. Its a fairly simple vb6 exe that is installed with the product. The settings are:
New Exe, installed with product.
Synchronous(Ignores exit code)
Deferred execution
install exec sequence - after install files

When it run, it throws and error -2753 and ends up bombing out. Any suggestions would be greatly appreciated.
