Did you try to open the file during or after the setup? Did the setup successfully create the registry entries? (You can check this with the Windows registry editor.)

Look for these keys:

HKEY_CLASSES_ROOT\.zzz
[(Default)]=zzzFile

HKEY_CLASSES_ROOT\zzzFile
[(Default)]=ZZZFile

HKEY_CLASSES_ROOT\zzzFile\shell\open\command
[(Default)]=Notepad.exe %1

HKEY_CLASSES_ROOT\zzzFile\shell\print\command
[(Default)]=Notepad.exe/P %1
